Bullion Booking Form - This is a sample of the form required to confirm your booking. Typically a purchasing client will call our toll-free bullion order line at 1-888-244-9999 to confirm pricing, availability, and shipping requirements and a J&M booking agent will complete this form and fax it to you for signing. Alternatively, you can call and confirm prices and fill in the form yourself with the provided prices and then sign and fax it to us.
Cash Declaration Form - This is a sample of the form required by USA and Canadian law for any clients purchasing or selling $10,000 or more worth of goods with cash. It is not required for drafts or bank transfers.
